Primary Number Holder*
The primary number holder is the wage earner, self-employed person, or main beneficiary who is entitled to benefits. The primary number holder can also be an individual applying for a new or replacement Social Security Number (SSN).
Claimant or Beneficiary (optional)
The claimant or beneficiary is the individual who is applying for benefits or receiving benefits on the primary worker’s record. This could be a wife, husband, child, widow(er), or ex-spouse of the worker.
